Privacy Policy

Diskin Life (“we”, “us”, “our”) is committed to protecting your privacy and handling your personal information with care and respect. This policy explains what information we collect, how we use and protect it, and the choices and rights you have. We handle personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs), and – because we provide health care – the Health Records Act 2001 (Vic) and its Health Privacy Principles.

1. The information we collect

Depending on how you interact with us, we may collect:

  • Personal details – your name, date of birth, address, phone number, email address and emergency-contact details.
  • Health information – your health history, symptoms, lifestyle, assessment findings, the care you receive and your progress. Health information is “sensitive information” under the Privacy Act and is given a higher level of protection.
  • Payment information – the details needed to process payments and, where relevant, health-fund or rebate claims.
  • Website information – if you use our contact or booking form, the details you enter; and general technical information your browser sends when you visit the site (such as your device type and pages viewed), which may be collected through cookies and analytics.

2. How we collect it

We collect most information directly from you – when you enquire, book, complete a form, attend the practice, or speak with us in person, by phone or by email. Where you have consented or where the law allows, we may also receive information from others involved in your care, such as your other health practitioners. We only collect health information with your consent and where it is reasonably necessary to provide your care.

3. Why we collect and use it

We use your information to:

  • provide, plan and review your care, and keep accurate clinical records;
  • communicate with you about appointments, your care and our services;
  • process payments and manage health-fund or rebate claims;
  • run and improve our Centre and this website; and
  • meet our legal, professional and insurance obligations.

We will only use your information for the purpose it was collected, a directly related purpose you would reasonably expect, or a purpose you have consented to.

4. When we disclose it

We do not sell your personal information. We may share it only where necessary, and in line with the law – for example with:

  • other health practitioners involved in your care, with your consent;
  • trusted service providers who help us operate – such as our practice-management, appointment, payment, IT and communication providers – who are required to protect your information; and
  • third parties where you have consented, or where we are required or authorised by law to do so.

5. Storage & security

We take reasonable steps to protect your information from misuse, loss, and unauthorised access, modification or disclosure – through secure systems, restricted access and staff confidentiality obligations. In line with Victorian health-records law, we retain adult health records for at least seven years after your last visit; records for anyone under 18 are kept until they turn 25. When information is no longer needed and we are no longer required to keep it, we take reasonable steps to destroy or de-identify it securely.

6. Accessing and correcting your information

You have the right to ask for access to the personal and health information we hold about you, and to ask us to correct it if it is inaccurate, out of date or incomplete. To make a request, please contact us. We may need to verify your identity first, and in limited circumstances allowed by law we may not be able to provide full access – if so, we will explain why.

7. Cookies & website analytics

Our website may use cookies and analytics tools to help the site work properly and to understand, in aggregate, how it is used so we can improve it. You can control or disable cookies through your browser settings; some parts of the site may not function as well if you do.

8. Marketing

If you have opted in, we may occasionally send you helpful updates or information about our services. You can opt out at any time using the unsubscribe link in any message or by contacting us – and we will not use your health information for marketing.

9. Overseas disclosure

Some of the service providers we use to run our practice and website may store data on servers located outside Australia. Where that is the case, we take reasonable steps to ensure your information continues to be handled consistently with this policy and Australian privacy law.

10. Complaints

If you have a concern about how we have handled your information, please contact us first – we take privacy seriously and will do our best to resolve it. You may also contact the Office of the Australian Information Commissioner (OAIC) or, for health-records matters in Victoria, the Health Complaints Commissioner (Victoria).

11. Changes to this policy

We may update this policy from time to time. The current version will always be available on this page, with the “last updated” date shown above.
